What to Expect
Pathstone has been listening closely to the voice of our clients and families and are committed to providing equitable access to services in a timely fashion. We have heard you and you have let us know that you want us to consider your voice and your choice in which services and supports best match your families needs and goals. As a result, Pathstone has implemented the Choice and Partnership Approach (CAPA) model to services. All aspects of the approach are family-centered. This means, Pathstone will follow YOUR lead to ensure you get what you need when you need it. We are increasing accessibility, supporting your choices, and reducing wait times!
CAPA is a model that combines collaborative approaches to practice to enhance effectiveness, and through continuous quality improvement manage wait lists.
CAPA brings together your service providers in an active partnership with you, our clients and families, so that services can then:
- Have clear working goals developed with the clients and families
- Match clients and families with the right people (therapists and programs that are best matched to the client/family's goals and objectives)
- At the right time without extended wait times for services
It is flexible to meet clients and families where they are with services that best meet their needs. It improves services by focusing on:
- Engagement
- Therapeutic alliance
- Client and Family choice
- Client and Family strengths, goals and care planning
- Improving access by ensuring timely appointments
- Ensuring clients and failies are seen in programming that matches their needs
